Please include contact info, web address and a lyric sheet. If your song is not published under ASCAP, BMI or SESAC then you will need to download and fill out KVRK's Copywrite Statement and Release Form. This will be required before your music can be processed for airplay.

After the form is completed and signed by the legal owners of the music, it can be faxed to us at (214) 351-6809 or mailed to the address above to the attention of Drue.
